Need a Replacement Course Card?

Please fill out the form below to request a replacement card for an AHA course you completed through Life Safety Solutions or an AHA Instructor aligned with our Training Center. All replacement card requests will be verified by Life Safety Solutions. A verification code will be sent to you via email along with a link to purchase the replacement card by credit card. If you prefer to pay by check: once you receive the verification code, include the code on your check and mail it to Life Safety Solutions.

Replacement Card:

• AHA BLS Provider Card:  $15

• AHA Heartsaver Card:  $25

(Note: Oct 2017 - AHA increased price of all Heartsaver cards: First Aid CPR AED, CPR AED, First Aid, Pediatric First Aid CPR AED) 

Requests are verified and codes/payment links are mailed out during business hours, Mon-Fri, 9am-5pm. Once your payment is received, we will mail your replacement card.

If we cannot verify your card request, we will contact you by phone or email to let you know.

Note: If you were issued an eCard instead of a paper card, we will resend your eCard notice to the email address you provide, once we verify your course completion.
